injury, damage, or liability sustained while participating. School will not be held liable for any misfortune that

By signing below, you are agreeing to the above and are certifying that you are in good health and have
consulted a doctor before beginning this dance program.
Please Note: 1.Tuition for each 10-weeks term (Saturdays) is due in full before the first session of instruction.
